Disguises that completely cover your face will still get enemies suspicious. 
Only if you are blending in with a crowd can you do that. However they still have the rather broken system of turning your back on NPC's won't stop them from getting suspicious. Only enemies with the same disguise as you can see through it. The distance is however much shorter, to ~10 feet. the time for them to detect you is still the same on normal difficulty.
Around December of last year, Square Enix did tweak the time to get detected for professional difficulty.
